Smiles Change Lives (SCL) has been providing access to orthodontic care for qualified children since 1997. Due to growing demand, we are now a nationwide program.
What are the benefits of becoming an SCL provider?

- We list you on our website as a provider.
- We issue letters to your referrals regarding your participation in SCL.
- We will contact you regarding press releases, events and other marketing opportunities.
- We send you SCL branded materials to advertise your participation in your office and on your website.

How does the process work?
- You determine how many approved SCL cases you would like to treat annually. The national average is three cases per orthodontist, per practice.
- Download and sign the provider agreement. Fax it, along with a copy of your license and insurance to 816-421-3008.
- Provide us with a list of your referring dentists on your provider agreement so we can reach out to them about your participation in the program.
- You will receive an SCL marketing "starter kit" in the mail.
- SCL will contact you to place screened, deserving patients.
